Direct Debit Suspension Form

Please fill in this form if you require your lessons to be suspended (outside of the 3 weeks at Xmas and 1 week at Easter)

All Dolphin Academy clients paying by direct debit are entitled to a minimum of 1 week to a maximum of 4 weeks suspension per calendar year. There is no charge for the first and second week and a charge of $5/lesson for the third and fourth week.

Please be aware that suspensions do not reduce the minimum term of the direct debit contract and any time suspended, is not included in the minimum term. All direct debit suspension requests require 10 days notice to be processed.

A suspension covers all students and lessons on the account and we are unable to suspend part of a payment. NB: Suspension requests will not be back dated.
